To make an appointment, please call our office at 662-205-4652. We are open Monday through Friday, 8:00 a.m – 5:00 p.m. If you are unable to keep a scheduled appointment, please contact our office by phone to cancel as soon as possible to avoid a no-show charge.
If this is your first visit to Baptist Medical Group-Elite Medical please bring a photo ID, insurance cards, and all medication you are currently taking including any over the counter medications and/or supplements. New patient registration forms may be downloaded below. Please bring completed forms to your first visit.
New Patient FormWe accept most major insurances, but please contact us at 662-205-4652 if you have questions regarding insurance coverage. All copay and deductible amounts are due at the time of service. Please review your individual plan prior to your first appointment to verify network status and copay and deductible amounts.
We will be happy to assist in the event you need a copy of your medical records or records need to be sent to another provider or entity. All requests should be made in writing and appropriate consent may be required depending on the nature of the request. Also a medical record fee may be charged for certain requests.
